How can I list more items in the Equipment Inventory?

If you have more than 20 equipment items: 

  • List them on a separate form (Excel or Word) and attach the document to the payment document. Do not input information in the Equipment Inventory section. 
  • In the Internal Comments, mention that the Equipment Inventory is attached. 
  • Check the box at the bottom of the Equipment Inventory section to indicate that the form is attached and will be forwarded manually RCUH Central Office.
